Dear Miss Shen Family:

Attached please find the 2008 registration form for the upcoming Miss Shen Softball Season. The registration fee is $60 for one player, $105 for two or more players, if post-marked by January 31, 2008. There is a $10 late fee per player for registrations post-marked after January 31, 2008. The Registration Fee will only be refunded for documented illness, injury or relocation. All refunds requested and approved after the uniform order is placed will be the amount paid less the cost of the visor and personalized shirt.

Also, any player whose registration is not post-marked by January 31, 2008 will not be guaranteed placement on a team and will not receive their name or number on their jersey. Registration forms are also available at the Clifton Park Town Hall. Please tell your friends about our program.

Please take the time to fill out the entire form. Please pay special attention to shirt information including size, name and number. Failure to provide a shirt size or name on the shirt will result in the player not receiving a personalized shirt.

It is very important to specify your e-mail address on the top of the form. Future Miss Shen communications will be sent to you via e-mail. Also, if your residence is within the Shenendehowa School District please indicate on the form by checking ‘Yes". Sign and date the form and mail it along with a check made out to "Miss Shen Softball" to:

You will receive information on your new team from your coach by mid April. Girls are assigned a team within a division based on their age as of December 31 of the year prior to the recreational season.

Attention girls playing on a travel team: All girls who are playing on a travel team are required to register and pay the registration fee. The money paid last fall was for your travel fee and fall ball fee. Girls who are 16 years old by January 1 or who play a spring sport for their school can opt not to play in the recreation league, but must still register and pay the registration fee. Please indicate this on the registration form.

The Miss Shen recreational season runs from April through June. Please note: all girls are required to provide/wear their own helmet with face mask. For additional information about the Miss Shen Program please visit our web site at http://www.missshen.org.

Any person having a check returned for insufficient funds will be charged a fee of $25, which must be paid before this registration is accepted.

I give permission to this league, its officers or representatives to provide medical treatment to the above named girl in case of emergency or injury. In the event of an emergency, I also give my consent that treatment or tests can be provided by any medical staff member on duty.

I do hereby acknowledge that I am required to support the operation of the snack bar and preparation of the fields as assigned by the coach or his/her designate.
